Please make certain to print out all the pages, complete the entire form,
sign and date it before submitting it to the camp office.
A $300.00 DEPOSIT IS DUE WHEN SUBMITTING A REGISTRATION FORM.
NO REGISTRATION FORM WILL BE PROCESSED PRIOR TO RECEIVING PAYMENT.

Must be completed entirely, signed and dated by a parent or
legal guardian and by the camper's doctor.
Must be submitted to the camp office prior to the start of camp.
NO CAMPER will be allowed to attend unless this is received by our office.
No Exceptions. |
